What is Social Prescribing?

The Mount Forest Family Health Team is starting a Social Prescribing Pilot Project in 2023.

If you feel this program is right for you, please speak with your doctor, nurse practitioner or other health care provider at the clinic.

What’s the process of the program?

• start with a conversation

• your provider will make a referral

• meet with a link worker to discuss options

• choose what is right for you

• start your ‘prescription’

• have a follow up with your health care provider to discuss your progress in
  4-6 weeks
